[Pharmacology and Toxicology] This drug is first converted into 5-fluoro-2-deoxyuridine monophosphate in the body. The latter inhibits thymidylate synthase, blocking the conversion of deoxyuridine monophosphate to deoxythymidine monophosphate, thereby inhibiting DNA biosynthesis. In addition, by preventing uracil and orotic acid from being incorporated into RNA, it achieves the effect of inhibiting RNA synthesis. This drug is cell cycle-specific and mainly inhibits S-phase cells.
[Pharmacokinetics] This drug is mainly metabolized in the liver and decomposed into carbon dioxide exhaled through the respiratory tract. [Usage and Dosage] There is a great difference in dosage between intravenous injection and intravenous infusion of fluorouracil. The general single dose for intravenous injection is 10-20 mg/kg per day based on body weight, used continuously for 5-10 days, with each course being 5-7g (even up to 10g). If administered by intravenous infusion, it is usually 300-500mg/m² per day based on body surface area, used continuously for 3-5 days, with each infusion lasting no less than 6-8 hours; continuous infusion can be maintained for 24 hours using an infusion pump during intravenous infusion. For primary or metastatic liver cancer, intra-arterial catheterization is often used. Intraperitoneal injection is given at a dose of 500-600mg/m² once per week, with 2-4 administrations constituting one course.
[Adverse Reactions] 1. Nausea, loss of appetite, or vomiting. Usually not serious with normal doses. Occasionally oral mucositis or ulcers, abdominal discomfort, or diarrhea may occur. A common side effect is reduced white blood cell count in peripheral blood (mostly reaching the lowest point 2-3 weeks after the start of treatment, and usually returning to normal after 3-4 weeks), platelet reduction is rare. Extremely rare cases include coughing, shortness of breath, or cerebellar ataxia. 2. Long-term use may lead to neurological toxicity. 3. Rarely, ischemia of the heart muscle may occur after medication, which may present as angina pectoris and changes in the electrocardiogram.
