With the arrival of summer vacation, many people want to take advantage of the holiday to deal with scars left from childhood. As a result, scar repair has become a focus this summer. However, even small scars near joints aren't easy to remove. We'd like to share some experiences in scar treatment. Not all scars can be removed, so experts at the Han Family Scar Repair Center have outlined several conditions under which scars can be repaired. Generally, scar treatment can be divided into three stages: preventing deformation, reconstructing active functional zones, and reconstructing passive functional zones.
Preventing Deformation: Wound contraction is part of the healing process. Prevent abnormal continuous wound contraction (contracture).
Methods: Reduce tissue deficiency. The correct diagnosis of tissue deficiency is the most important part of reconstruction work - release before supplementing. Methods include suturing, excision followed by suturing and Z-plasty, and skin grafting (partial-thickness, full-thickness, composite). Selection of donor sites follows the proximity principle. Flap reconstruction includes flaps, muscle flaps, microflaps, and tissue expanders (water balloons).
Suggestion: Tissue expanders can be used for localized sheet-like scars, requiring two surgeries spaced about two and a half months apart.
Types of Scars: Wide scars, hypertrophic scars, pigmentation issues (due to poor circulation and venous return in limb scars, besides the scars themselves, there are also likely pigmentation changes), keloids.
Problems with Scars: Itchiness, pain or infection, and scar contracture leading to functional impairment.
Delayed Healing: Unstable skin
Wound Treatment: Artificial skin coverage/fixed burn ointment; just healed wounds → anti-inflammatory ointment/lanolin oil for 4~5 days. Sunscreen, antipruritic drugs...
Suggestions:
1. For excessive scar hyperplasia, consider local steroid injections to improve the appearance of scars.
2. Depressed scars can be treated with autologous fat injection.
3. Raised scars treated with flaps can be further refined with liposuction.
4. For well-healed wounds, early proper use of cosmetic glue and silicone sheets can prevent scarring.
5. "Pressure therapy" refers to using various materials that provide pressure (elastic pressure garments, silicon pads, compression sleeves...).
6. Avoid scratching the wound after surgery to avoid permanent scarring.
Elastic Pressure Garments Usage Suggestions:
Pressure garment wear: 2 weeks ~ 2 months after wound healing
Ideal pressure: 15~25mmHg, 22~23 hours per day, (every 2~3 months)
Low but persistent pressure → better results!
High pressure → faster scar maturation
If after removing the pressure garment for 30 minutes, there is still a pressure mark, it indicates it's too tight. Completely take off only for bathing, moisturizing the skin, exercise.
Stop wearing when: the wound turns white, flat, soft.
Silicone Sheets (Silicon, Gel Pads): Composites containing silicon elements
Composition: Liquid, gel, solid
Usage time: 12~24 hours/day
Advantages: Reduces pain, prevents skin graft contraction, softens scars, reduces scar hyperplasia.
Disadvantages: Allergic reactions causing skin irritation → stop usage
Scar formation opportunities and recommendations for pressure garment use:
6~2 days of healing: 0% chance of long scars → no need for pressure garment
10-14 days: 15% chance → preventive for non-Caucasians
14-21 days: 85% chance → therapeutic recommended
>21 days: 100% chance → therapeutic necessary
In the first three months, if the texture hasn't changed but the color gradually fades → no excessive scar hyperplasia; if red blood vessels are numerous on the side (raised) → scar hyperplasia.
Conclusion: Proper wound handling after injury involves good inspection during suturing, debridement management, maintaining wound moisture, and absolute rest, all of which help reduce scar formation. For laser scar removal, linear wounds are most effective, and treatment can begin around 6 weeks after good wound healing.
