Symptoms of Diabetic Eye Disease
Retinopathy
Retinopathy is the most common major diabetic eye disease, often leading to vision loss or blindness. According to statistics, about 50% of patients with a diabetes duration of around 10 years may develop this condition, and it rises to 80% for those with over 15 years of diabetes. The more severe the diabetes and the older the patient, the higher the incidence. This condition results from microvascular disease due to diabetes, causing damage to the capillary walls of the retina. Combined with high blood coagulability, it easily leads to thrombosis, stasis, and even vessel rupture. A report from the U.S. National Diabetes Commission states that diabetic patients are 25 times more likely to become blind compared to non-diabetics. Currently, diabetic retinopathy has become one of the top four causes of blindness after age-related macular degeneration. However, early treatment still yields relatively ideal results, and the cost of early prevention is much lower than that of late-stage treatment. Therefore, it is particularly important for diabetic patients to prevent eye diseases, which can effectively prevent or delay the onset of eye problems.
Cataracts
Diabetes-induced cataracts account for as much as 60% of all cataract patients, with a significantly higher surgery rate than other types of cataracts, usually requiring surgical treatment. Animal experiments have proven that high blood sugar levels can lead to cataracts both in vivo and in vitro. This must be taken seriously.
Unstable Refractive Errors
Many patients initially do not know they have diabetes. After some time, they might notice sudden nearsightedness, difficulty seeing far but clarity up close, indicating diabetic eye disease. Patients may then get glasses, which work well at first but soon their vision returns to normal. If blood glucose levels are checked repeatedly, there will be peaks and troughs, showing fluctuating blood sugar increases. When blood sugar rises sharply, patients may suddenly shift from normal vision to nearsightedness or experience relief from presbyopia. The mechanism involves elevated blood sugar affecting ocular structures. When blood sugar drops, vision may return to normal or require reading glasses again. Thus, fluctuating blood sugar can cause variable changes in vision.
Open-Angle Glucose
Diabetes can easily lead to open-angle glaucoma, also known as chronic simple glaucoma. This type of glaucoma is more common in middle-aged and older adults but can also occur in younger individuals. It is usually bilateral, with a slow onset, gradually increasing intraocular pressure while the angle remains open. Often, there are no obvious symptoms until significant vision and field loss occurs, making early diagnosis crucial. Initially, there may be no noticeable discomfort. As the condition progresses, there may be mild headaches, eye pain, blurred vision, and rainbow visions that resolve with rest, often mistaken for visual fatigue. Central vision may remain stable for a long period, but peripheral vision defects can appear early. Eventually, prolonged high intraocular pressure causes optic nerve atrophy, shrinking and disappearing fields of vision, leading to blindness. Throughout the course, there are usually no obvious external signs except for slight pupil dilation and iris atrophy in later stages.
