This includes areas such as the anus, vagina, nasal cavity, ear canal, as well as physical therapies like sunlight baths, sand therapy, wax therapy, hydrotherapy, fumigation, etc.
External Treatment Methods:
Anesthetic Acupoint Block Method: Often used for patients experiencing severe pain. Commonly selected is a fan-shaped injection of 0.5%-1% procaine or 0.25% bupivacaine at the Changqiang acupoint, each time 5ml. The treatment is repeated every other day, with five sessions making up one course of treatment.
Chinese Medicine Injection Method: Often uses drugs with blood-activating and stasis-resolving, pain-relieving, and hemostatic effects for local blockage of anal fissures. By relieving internal sphincter spasm, improving local circulation, it promotes wound healing.
Fumigation and Local Dressing Change Method: Used for anal fissures in all stages. It employs anti-inflammatory, sterilizing, blood-activating, swelling-reducing, and muscle-growing drugs for fumigation and dressing changes. Acupuncture Method Suitable for Severe Anal Fissure Pain: Can treat simple anal fissures, main acupoint being Changqiang. Insert about an inch, strong stimulation 2-3 times, leave needle for 2-5 minutes. Supporting acupoints Baihuan Shu, insert 2-3 inches, rotate strongly, leave needle for 2-5 minutes. Seven sessions make up one course of treatment. Through stimulation of meridians and acupoints, it unblocks meridians, regulates qi and blood, improves local nutritional conditions, thus achieving pain relief, hemostasis, and promoting healing.
Acupoint Embedding Therapy: Uses the method of embedding catgut sutures at the Changqiang acupoint, thereby achieving the purpose of treating anal fissures. Divided into puncture embedding and suture embedding methods. The embedded thread here can produce a stimulating effect, inhibiting local pain sensation impulses, simultaneously having a significant regulatory effect on the anal sphincter, and improving local blood circulation, thus achieving the therapeutic purpose.
Cauterization Method: Using high heat to cauterize the anal fissure wound, after the eschar falls off, a fresh wound forms which is easier to heal. Generally, electrocautery or laser can be used. Lie on your side, routine disinfection, local anesthesia, then use an electrocautery or laser device to accurately cauterize the anal fissure wound, carbonizing it. Afterward, include an oily gauze strip in the wound and fix it with dressings. Post-surgery, sit in a bath after defecation, include one hemorrhoid suppository in the anus.
Cryotherapy: Generally uses liquid nitrogen to freeze the anal fissure wound, temperature usually at -160 degrees Celsius, freezing each time for 20-30 seconds, repeating 3-4 times. Include an oily gauze strip in the wound and fix it with dressings. Post-surgery, sit in a bath after defecation, include one hemorrhoid suppository in the anus.
Chemical Sphincterotomy: Modern research confirms that through non-adrenergic non-cholinergic pathways, nitric oxide can cause relaxation of the internal anal sphincter. Local use of nitric oxide donors can reduce anal canal pressure, known as chemical sphincterotomy. A commonly used method is applying 0.2% nitroglycerin ointment locally for treatment, which has a high healing rate, is cost-effective, and has no aftermath of causing fecal incontinence, but has side effects such as headache and anal burning sensation.
