Perianal abscess is a common anorectal disease, mainly caused by acute suppurative infection occurring in the soft tissues surrounding the anal canal and rectum or in the spaces around them, forming an abscess, which is referred to as perianal and perirectal abscess.

According to the Yangzhou Armed Police Hospital Anorectal Center, perianal abscess is an acute anorectal disease characterized by spontaneous rupture or often forming an anal fistula after surgical incision and drainage. The most common type is the perianal subcutaneous abscess, usually formed by the spread of anal gland infection through the subcutaneous part of the external sphincter or directly outward. When a perianal abscess occurs, it generally presents with the following symptoms:

1. Subcutaneous perianal abscess: Mainly characterized by pain, initially as distension pain, then throbbing pain when pus forms, and worsens during defecation. Abscesses located in front of the anus can cause urinary retention, while those behind the anus cause tailbone pain. General systemic symptoms are mild, but local swelling, redness, tenderness, and fluctuation may be present.

2. Ischiorectal fossa abscess patients: Present with systemic discomfort, fever, chills, and elevated temperature. Locally, there is swelling on one side of the anus, redness, burning pain, throbbing pain, tenderness, restlessness, worsening pain during movement and defecation, and difficulty urinating.

3. Pelvic rectal fossa abscess patients: Severe systemic symptoms include initial chills and high fever, general fatigue, and severe cases may have sepsis-related toxic symptoms. Local symptoms are mild, only showing a sensation of rectal prolapse, soreness, or discomfort, and may also cause difficulty urinating.

4. Retrorectal abscess: Systemic symptoms are similar to pelvic rectal fossa abscess, but local symptoms primarily involve sacroiliac waist soreness and heaviness, radiating to the back and both thighs, with tailbone tenderness, preventing the patient from sitting upright.

5. Submucosal rectal abscess patients: Present with general discomfort, fatigue, and fever. Locally, rectal irritation symptoms predominate, such as tenesmus, a feeling of heaviness, increased bowel movements, or a sense of needing to defecate.

6. Tuberculous perianal and perirectal abscess: Different from the aforementioned bacterial infections, patients often develop chronic conditions over several weeks or months before an abscess forms. Local pain is not severe, accompanied by low-grade fever, and local redness and swelling are not obvious.

Experts at the Yangzhou Armed Police Hospital Anorectal Center point out that perianal abscesses typically start with pain before forming an abscess. Since acute perianal abscesses often lead to anal fistulas, many people fear surgery due to concerns about hematochezia causes. However, experts warn against this misconception; untreated perianal abscesses will eventually lead to anal fistulas, necessitating surgery later on. Therefore, early treatment of perianal abscesses is recommended to avoid complications.
