Clinical observation of radiofrequency ablation of uterine fibroids: A loop is formed, and then by the diffusion electrode, electrically charged ions in the surrounding tissue of the electrode needle are impacted by the current, causing oscillations which result in biological heat. This causes a local temperature rise. When the temperature exceeds 45~50℃, the protein within the tumor cells denatures, the double-layer lipid membrane dissolves, the membrane cracks, intracellular and extracellular water loss occurs, resulting in coagulation necrosis of the tumor tissue, which is eventually excreted or absorbed by the body. In this group, 142 cases of uterine fibroid treatment were observed with a total efficiency of 98.7%. Patients were able to get out of bed and walk afterward and were followed up for 6 months with no adverse reactions or complications.
Radiofrequency ablation surgery understandings:
(1) Selection of the puncture point: When fibroids are ≤3cm, the puncture point is in the middle of the leiomyoma. For fibroids larger than 5cm or more, the puncture site should be selected on the side of the tumor, and then the other side can be punctured. Graded puncture radio frequency treatment can achieve more thorough myoma coagulation; or use of drugs before RF to decrease the tumor size for improving the RF treatment.
(2) Puncture depth: Self-curing knife can be used under B-ultrasound guidance to puncture into the center of the tumor. The self-curing tip from the serosa should be greater than 1.2cm.
(3) Patients should pay attention to the observation and handling of complications such as abdominal pain, vaginal bleeding, fever, uterine perforation, and infection.
(4) Contraindications for the treatment include reproductive system inflammation or malignancy, serious heart, lung, liver, brain, and kidney dysfunction, and patients with moderate or extreme anemia.
In short, radiofrequency ablation is a local physical therapy that is safe, reliable, minimally invasive without damage or minimal damage to surrounding tissues, simple and easy to operate. It retains the uterus and does not affect ovarian function, making it clinically therapeutic valuable.
