Women are more prone to anal fissures than men, especially middle-aged women who have given birth. The incidence of anal fissures in women is higher than in men, particularly among young women. According to statistics from the Japanese Colorectal and Anal Society, 18% of anal fissure cases occur in women compared to 11% in men. Experts from the Yangzhou Armed Police Hospital Anorectal Center point out that anal fissures are a common and frequently occurring anorectal disease. They can occur at any age, but are more prevalent in women than in men. The primary cause of anal fissures is dry stools and constipation.
What are the symptoms of anal fissures in women?
1. Pain. Pain is the main symptom of anal fissures in women. When there is an urge to defecate and the anus stretches, the pain begins. During defecation, as feces impact the fissure, there is an immediate sensation of burning or cutting pain inside the anus, known as defecation pain. After a few minutes, the pain may subside or stop, but then due to anal sphincter spasm and contraction, the patient experiences severe pain again.
The severity of the pain varies depending on the size and depth of the fissure. This period of pain often lasts from half an hour to several hours, causing the patient great discomfort. When the sphincter becomes fatigued from spasms, the pain gradually stops, marking one cycle of pain. Subsequently, during defecation or sneezing, coughing, or urinating, cyclical pain may recur.
2. Constipation. Women with anal fissures often avoid defecating out of fear of pain, leading to constipation. Constipation can worsen the anal fissure, creating a vicious cycle.
3. Bleeding. Generally, female patients experience minimal bleeding. There might be a few drops of fresh blood during defecation or streaks of blood on the feces. The amount of bleeding correlates with the size and depth of the fissure; larger and deeper fissures result in more bleeding, while smaller ones produce less.
4. Itching around the anus. Many female patients with anal fissures experience itching around the anus due to secretions irritating the skin. This can lead to perianal dermatitis. Chronic conditions may include malnutrition, anemia, and weight loss. During acute episodes, there may be fever, systemic toxicity symptoms, poor appetite, vomiting, bloating, and dehydration.
Experts from the Yangzhou Armed Police Hospital Anorectal Center warn that anal fissures are a common and frequently occurring anorectal disease. Many women mistake anal fissures for hemorrhoids, which can lead to misdiagnosis and inappropriate treatment. Therefore, women experiencing the above symptoms should seek professional medical evaluation promptly to prevent worsening of the condition.
