4. Poor drainage of the abscess cavity, or the external opening becoming smaller, intermittently closing and breaking open again, causing pus to accumulate in the cavity, leading to the recurrence of an abscess which then perforates and forms new branches or fistulas.
5. The fistula passes through the anal sphincter muscle at different heights, and the contraction of the sphincter muscle hinders the discharge of pus, resulting in poor drainage.
