Conservative drug therapy can be used. If the bleeding cannot be controlled, sclerotherapy injection may be an option. This method causes less pain and is more easily accepted by patients. However, if the condition involves polypoidal hemorrhoids, early removal is recommended.

In Stages II and III, besides bleeding, there is also prolapse of lumps outside the anus after defecation. These lumps may either retract spontaneously or require manual assistance. This stage is the best time for surgical treatment because compared to Stage IV, surgery at this stage causes less suffering. In Stage IV, although there is less rectal bleeding, there is prolapse that cannot be retracted even with manual support. Symptoms at this stage are serious and affect daily life. However, this stage sees the most clinical surgical treatments.

Actually, many hemorrhoid patients have misconceptions about surgical treatment. First, they believe that hemorrhoid surgery is very painful and are very afraid of it, only opting for surgery as a last resort. They do not realize that surgery in later stages of hemorrhoids is relatively more painful. With advancements in hemorrhoid surgery, our techniques and methods have continuously improved and evolved.

From the old simple ligation to modern minimally invasive techniques, external stripping and internal ligation plus surgical anal dilation significantly reduce patient suffering. Secondly, some patients think that hemorrhoids are minor ailments and choose to delay treatment. In reality, delaying treatment harms oneself and may even miss the optimal treatment window. Hemorrhoids, when properly treated, can be fully cured without recurrence.

Simple anal fistulas are also prone to become complex anal fistulas in summer due to infections; diabetic patients should seek early treatment.
