The Process of Mandibular Angle Reshaping Surgery
Surgical Process
The surgery can be performed under general or local anesthesia, with routine disinfection and draping. An incision is made in the mucosa of the buccal-gingival groove anterior to the mandibular ramus, reaching directly beneath the periosteum of the mandibular angle, approximately 3-4 cm in length. Care must be taken not to injure the mental nerve emerging from the mental foramen. The attachment points of the masseter muscle are detached subperiosteally to expose the posterior border of the mandibular ramus, the lateral surface of the mandibular angle, the posterior edge, and the inferior border. According to the designed bone removal amount, a bone milling line is drawn; using a long-handled bone mill, the mandibular angle is milled according to the milling line. Typically, an arc-shaped area of about 3-4 cm in length and 1-2 cm in height can be milled, smoothing out the sharp edges of the mandible. Hemostasis is achieved before suturing the wound. If the patient has a relatively large cheek, part of the inner layer of the masseter muscle can be removed simultaneously, or some separation can be done laterally to remove part of the buccal fat pad. The fat of the buccal fat pad is slightly lighter in color, appears as large bubbles, and is very loose. When removing it, attention must be paid to hemostasis. After completion, the incision is sutured, and stitches are removed after 7-10 days. Mandibular angle resection surgery is relatively safe, but care must be taken to protect nerves, vessels, and ensure symmetrical bone milling.

Since the cheeks contain abundant nerves and blood vessels, and the mandible and temporomandibular joint play crucial roles in chewing and speech, mandibular angle reshaping surgery is one of the more challenging procedures in aesthetic surgery. Any mistake could lead to irreparable consequences. In the domestic beauty market, which is still unregulated and where doctors' qualifications vary greatly, individuals seeking surgery should carefully choose hospitals and surgeons. Due to significant differences in understanding and technical levels among plastic surgeons across the country, and even some beauty salons or clinics offering this surgery, the risks for patients have increased significantly.
