Experts from the Epilepsy Treatment Center of Guangzhou Bauhinia Hospital said that the diagnosis of epilepsy in adults should follow a two-step principle: first, confirm whether it is epilepsy; then, on the basis of confirming epilepsy, distinguish what type of seizure or epilepsy syndrome; finally, clarify the cause of epilepsy and try to make a correct estimate of the degree of disability of the epilepsy patient.
The diagnostic process for adult epilepsy by experts at the Epilepsy Treatment Center of Guangzhou Bauhinia Hospital:
1. Issues to note before diagnosis: Epilepsy has two characteristics, namely clinical seizures and epileptiform discharges on EEG. Only having epileptiform discharges on an EEG cannot diagnose epilepsy because about 1% of normal people may have epileptiform discharges on their EEGs. Epileptiform discharges can also occur in other non-epileptic diseases. Having only clinical seizures should not lead to a hasty diagnosis of epilepsy.
2. Clinical features and EEG manifestations of epilepsy: There are two main characteristics of clinical seizures in epilepsy:
(1) Commonality: The commonality of epilepsy refers to the common features shared by all types of epilepsy, which are episodic, transient, repetitive, and stereotyped.
(2) Individuality: This refers to the specific characteristics of different types of epilepsy, which are the main criteria for distinguishing one type of epilepsy from another. The combination of the commonality of epilepsy attacks and the individuality of specific types form the most important diagnostic basis for epilepsy.
EEG Manifestations: The EEG during and between epileptic seizures shows different manifestations, but the main feature remains the burst activity that stands out against the background. Clinically, the most common and characteristic are spike waves, sharp waves, spike-slow complex waves, sharp-slow complex waves, and paroxysmal rhythmic waves.
