(1) Change positions. Generally, turn over every 2-3 hours and massage the pressured areas.
(2) Use bedsore prevention aids. Place soft pillows at certain areas to help stabilize the body and relieve pressure on affected areas. Protect the back and bony prominences with air rings, cotton pads, sponge pads, or soft pillows. The main function of the mattress is to redistribute the body's pressure, avoiding concentrating the patient's weight on just a few points. Mattresses can be made of high-quality foam or mobile pressure point air cushions, increasing the contact area of the body and dispersing the pressure.
(3) Promote local blood circulation. Sunbathing or ultraviolet irradiation can be performed. During each turn-over, use a warm towel to wash the bony prominences under pressure. For dry skin, apply a small amount of moisturizer to prevent cracking and bleeding. When the skin is damp, use sanitary paper to absorb moisture and minimize friction as much as possible.
(4) Keep the sheets clean. Sweep the bed promptly to remove granular substances that may rub the skin. The sheets should be soft and not dragged. The method for changing the sheet is: place the patient's hands on their chest and abdomen and bend their legs. The person changing the sheet supports the patient's head and buttocks, moving them closer to themselves, then gently pushes the patient to one side while moving the pillow under the patient's head. When the patient's back is exposed, observe the skin color and pressure situation, then cover the patient well to avoid getting cold. Roll the dirty sheet onto the patient's body and brush it clean. Align the midline of the clean sheet with the midline of the bed, lay down the head first, then the foot, fold both ends into angles and tuck them under the mattress, then lower the rubber sheet. Finally, move to the opposite side of the bed and use the same method to turn the patient to the clean side. With both hands, wrap and pull out the dirty sheet, brush the mattress clean. Pull out the clean sheet and rubber sheet from under the patient and lay them according to the above method, finally laying the patient flat.
(5) Improve overall conditions. For nutritionally deficient hemiplegic patients, primary disease prevention and treatment for bedsores should be conducted, such as diabetes. If blood sugar control is poor, complications like skin edema, hypoxia, and sensory abnormalities may easily form bedsores. Therefore, after a clear diagnosis, the primary disease should be actively treated. For hemiplegic patients undergoing home treatment, caregivers should be educated about bedsore prevention knowledge.
(6) In the early stages of bedsore development, physical therapy treatments such as infrared, ultraviolet, and laser can be used. Local irradiation of the bedsore area can be done once daily, with six sessions constituting one course of treatment.
