1. Care for the Ecchymotic Reddening Stage (Grade I)
Firstly, various preventive measures should be taken (as previously mentioned) to prevent continued pressure on the affected area. Increase the frequency of turning and massage; apply local heat to improve blood circulation and increase resistance. For obvious redness and swelling, use 50% magnesium sulfate or 75% alcohol for local wet compresses to promote inflammation absorption and retreat. Generally, after 2-4 days, local symptoms can subside.
2. Care for the Inflammatory Infiltration Stage (Grade II)
(1) For severe redness and swelling (purple-red), strengthen nursing care and turn regularly. If turning is not allowed due to condition, use "three-horse air cushions," water pads, etc., to reduce local pressure and keep the skin dry.
(2) If blisters appear, use a sterile syringe to aspirate exudates from the blister, then apply 2.5% iodine tincture and cover with dry sterile gauze or apply compound balsam tincture, 1% gentian violet solution, 1% chlorhexidine, 1% neomycin, etc., after aspiration. Infrared radiation may also be used. Generally, recovery occurs within 2-5 days. Some methods include debridement followed by applying 2% iodine tincture and directing oxygen flow onto the wound (6-10L/min for 10-20 minutes, twice/day), which has shown good results.
3. Care for the Ulcerative Stage (Grade III)
At this stage, thoroughly clean the wound and remove necrotic tissue. After seeing the wound become rosy, repeatedly flush with physiological saline plus 80,000U gentamicin, apply pressure momentarily with dry gauze, then wrap with high-moisture multi-layered physiological saline gauze for continuous 2-3 days. Alternatively, apply iodine tincture to the wound, cover with 2-4 layers of wet gauze soaked in gentamicin (20,000U), externally apply erythromycin ointment, then wrap with sterile gauze, once/day, generally healing within 5-8 days. For deeper wounds with more secretion, clean the wound using 3% hydrogen peroxide, apply 2% iodine tincture post-debridement, then perform oxygen therapy, which shows better effects for Pseudomonas aeruginosa and E. coli infections. Post-debridement, wet dressing with furacilin or external application of furacilin powder can effectively reduce exudation and shrink the wound. For granulation tissue edema, wet dressings with 3% hypertonic saline or 50% magnesium sulfate solution can be applied 2-3 times/day; for poor granulation growth, consider wet saltwater gauze dressings or external application of glucose powder, myogenic liquid, etc. If there's fungal infection, options like clotrimazole, nystatin, or alkaline solutions can be used. For excessive or uneven granulation tissue, trim and reapply medication after cleaning.
4. Care for the Necrotic Stage (Grade III)
This stage is relatively serious. First, try to remove necrotic tissue and maintain smooth drainage. Post-debridement, apply external wet dressing with 0.5%-1% cetavlon gauze, once/day, which effectively reduces the wound size and accelerates granulation growth. Debridement followed by iodine tincture application, pressurized flushing with gentamicin, and 30-minute infrared radiation, then covering with gentamicin gauze and externally applying erythromycin ointment before wrapping with dry gauze, generally heals within 3-4 weeks. Using fresh placenta (cut into small pieces after removal), applied externally post-debridement and gentamicin flushing, can promote rapid granulation tissue growth. Chen Xiaoying et al. used 20% sodium chloride sugar mixed liquid wick for wet dressing, which quickly reduced purulent secretions and promoted granulation growth. For larger wounds, skin grafting can be considered after granulation tissue fills the wound.
