A mature follicle secretes more estrogen, causing congestion in the uterine lining and red blood cell leakage. It is also possible that when the fimbriae of the fallopian tube capture the egg, the blood-containing follicular fluid is transported back into the uterus via reverse peristalsis and then flows out through the cervical canal and vagina.

Research shows that for women who ovulate, about 60% have microscopic red blood cells found in the cervical mucus at mid-cycle, though no visible bleeding can be seen with the naked eye; this is not considered a pathological phenomenon. However, women experiencing ovulation bleeding must rule out other reproductive tract diseases such as irregular menstruation, cervical erosion, cervical polyps, cervical cancer, endometrial polyps, submucosal uterine fibroids, or endometrial adenocarcinoma.

If a doctor confirms ovulation bleeding without any organic lesions, it can be left untreated or managed with a small amount of estrogen supplementation or traditional Chinese medicine. If the bleeding is functional, hormone therapy can be used based on test results to regulate endocrine function or stop the bleeding using estrogen and progesterone, or traditional Chinese medicine can be used for symptomatic treatment.
