### Daily Washing

The secretion of smegma can occur quite rapidly, especially for those with a long foreskin or phimosis, where it accumulates quickly. If you don't wash for a day, it becomes very noticeable. It is generally recommended to clean 1-2 times daily. Many men choose to wash in the morning upon waking and again before bedtime. When washing, try to pull back the foreskin to ensure thorough cleaning. Usually, warm water is sufficient, but mild cleansing products can also be used to achieve a more thorough clean. However, those with phimosis often find it difficult to clean thoroughly.

It's crucial to wash carefully before and after sexual intercourse. Washing before sex is to protect your partner, avoiding the transfer of smegma into the vagina, which could lead to health issues for women. Washing after sex is to protect the man, preventing bacteria or other microorganisms brought from the vagina from growing and multiplying under the foreskin, which could lead to inflammation.

### When Surgery is Necessary

Does a long foreskin always require surgery? Not necessarily, as undergoing surgery can be painful. As long as it does not affect the normal development of the penis or urinary function, and there are no repeated occurrences of balanoposthitis, it should not be considered a disease. In such cases, it is best not to undergo surgery and instead adopt an observation and waiting approach. Phimosis, however, is different from a long foreskin. If you have phimosis, it is advisable to seek treatment actively and consider surgery. This is because the glans of individuals with phimosis are tightly bound by the foreskin, limiting proper stimulation and potentially restricting the development of the glans. Urination may also be abnormal, and even sexual activity could be affected. Even when attempting local cleaning, it is often difficult to clean thoroughly. Phimosis can also lead to paraphimosis.

### Circumcision

Circumcision is the primary method hospitals use to handle long foreskins. The methods commonly used include traditional surgical removal, laser treatment, and circumcision rings. Each method has its advantages, and the specific choice depends on the doctor's decision and the patient's preference.
