One: Family Planning Service Certificate (Birth Permit)
Tianjin birth permit application time: Early pregnancy
Function: The family planning service certificate is what we commonly refer to as the "birth permit." This is the first document for having a baby, so it should be processed as soon as you find out you're pregnant. Don't think this certificate is optional; it's actually the legal "pass" for your baby to enter the world. Later on, your baby’s birth registration, household registration, and other welfare benefits will all be closely related to it.
Required materials: When applying for the certificate, bring your household registration book (preferably both spouses', but if they are not in the same book, bring the mother's), both spouses' ID cards, and proof of being married and having no children (this can be issued by your workplace or local residents' committee). Go to the street office to apply for the birth permit.

Medical Birth Certificate
After waiting for 10 months, the end is near. Your baby is about to be born, and when you check into the hospital, the hospital will require you to fill out a self-completion form for the Medical Birth Certificate, preparing for the upcoming baby's birth certificate. The birth certificate is your baby's first life record.
The self-completion form generally includes the following information: parents' names, ID numbers, ethnicity, baby's name, baby's registered address, mother's residential address, bed number, etc. If you haven't decided on a name for your baby when you are admitted, you can use a nickname temporarily. However, before discharge, you must give your baby an official name, otherwise some hospitals will not issue the birth certificate.
Tips:
* Be very careful when filling out the self-completion form for the Medical Birth Certificate, because once filled out and printed, it cannot be changed.
* Carefully check the Medical Birth Certificate upon receipt. If any errors are found, promptly request a replacement from the hospital. The birth certificate must not be altered; once altered, it is considered invalid.
* The Medical Birth Certificate is the baby's valid legal document. Keep it safe.
b. Pregnancy diagnosis certificate and laboratory test results issued by the designated hospital (with the fertility stamp);
c. One 1-inch uncapped photo of the female.
Two: Medical Birth Certificate (Birth Certificate)
Tianjin birth certificate application time: Upon discharge
The birth certificate is your baby's first life record and can be obtained at the hospital.
How to apply: After waiting for 10 months, the end is near. Your baby is about to be born, and when you check into the hospital, the hospital will require you to fill out a self-completion form for the Medical Birth Certificate, preparing for the upcoming baby's birth certificate.
Details to note:
* Be very careful when filling out the self-completion form for the Medical Birth Certificate, because once filled out and printed, it cannot be changed.
* Carefully check the Medical Birth Certificate upon receipt. If any errors are found, promptly request a replacement from the hospital. The birth certificate must not be altered; once altered, it is considered invalid.
* The Medical Birth Certificate is the baby's valid legal document. Keep it safe.
Special Note: Before applying for the "Medical Birth Certificate," make sure you have already chosen a name for your baby.
When naming your baby, avoid deliberately picking obscure characters from the dictionary. This way, you can avoid many troubles, and others won't have to ask how to pronounce the character or even mispronounce it. Nowadays, registration or examination enrollment uses computer information systems, and too obscure characters may cause typists to spend a lot of effort searching in the font library or may not even find the character in the system, which could lead to a lot of inconvenience.
Three: Household Registration
After the baby is born, there is one more family member at home. According to the household registration regulations, it's time to register the baby's household registration, making him/her officially a part of the family under the law.
Essential documents: To register the household, necessary certificates must be prepared. According to current urban and rural household registration regulations and family planning management rules, the required certificates include: the birth permit issued by the family planning department, the birth certificate issued by the hospital, and the household registration book.
Steps: When registering the household at the police station's household registration office, carefully fill out the household application form, complete the household registration, pay a certain fee, and your baby's name will be added to the household registration book.
Note: Citizens' rights start at birth, and newborns' birth registration is their first household registration, which is very important for the future, so parents must not neglect this matter.
Detailed matters:
Infants born after marriage, before marriage, unplanned births, and abandoned infants should all be registered.
Within one month of birth, the infant's father, mother, or other guardian, neighbor, with the birth certificate or community (village) residents' committee proof, should register with the household registration authority where the infant usually resides (i.e., the usual residence of the infant's parents). For people with different place household registrations who get married, the child can be registered in either parent's location.
For babies born at home:
If a baby is born at home, first, the parents need to apply to the community or village residents' committee for a birth certificate. When applying, prepare both parents' ID cards, the family planning certificate from the registered location, proof from the midwife, and proof of parent-child relationship. Once all the relevant procedures are completed, a birth certificate can be issued retroactively, and then the household registration can be reported directly to the police station.
Four: Only Child Certificate
After having a baby, if there are no plans for more children, consider applying for an only child certificate at the local residents' committee. After obtaining the certificate in Tianjin, from the day of issuance until the child turns 14, parents can receive a monthly bonus of 10 yuan, and retirement salary can increase by 5% (according to each province and city's family planning regulations, the rewards and distribution methods for only children may vary by region).
Application process:
1. Residents fill out the application form - 2. Initial review by the unit or residents' committee - 3. Examination by the family planning office - 4. Approval by the district family planning commission - 5. Issuance of the certificate and reward
Required materials:
1. Original couples: Both spouses' and the child's household registration proof, marriage proof;
2. Divorced individuals: Eligible party and the child's household registration proof, divorce proof;
3. Widowed individuals: Eligible party and the child's household registration proof, widowed proof;
4. Remarried individuals: Both spouses' and the child's household registration proof, divorce proof, marriage proof.
Five: Other documents to be processed after the baby is born
Whether it is "BCG vaccination," "Vaccination Certificate," or "Health Check-up Card for Children Aged 0-3," these are all essential documents for daycare and kindergarten admission. Timely health checks and vaccinations for your baby are very important, so mothers shouldn't slack off!
(1) BCG Vaccination Record
BCG vaccine is made from non-pathogenic bovine tuberculosis bacilli cultured artificially. Babies weighing over 2.5 kg, gestational age over 37 weeks, without fever, diarrhea, etc., can receive BCG vaccination within 6-24 hours of birth. If the vaccination is done more than 3 months after birth, a tuberculin test is required, and only those with negative results can be vaccinated.
Additionally, after birth, eligible newborns will also be injected with the first dose of hepatitis B vaccine.
(2) Vaccination Certificate
The vaccination certificate is an essential document for children entering daycare, kindergarten, and school. Therefore, within one month of the baby's birth, parents should take the baby's first dose of hepatitis B vaccine and BCG vaccination record proof to the jurisdictional disease prevention and control center at the registered address (if registered elsewhere and living locally for more than 3 months, apply at the local residence) to obtain the child's vaccination certificate; rural children should apply at the township health clinic's immunization outpatient department. This allows timely administration of the second dose of hepatitis B vaccine and other corresponding vaccines. The vaccination certificate will specify all required vaccinations within the prescribed scope and detailed matters during vaccination.
Tip:
If a child has not completed the specified immunizations due to migration, travel, or temporary residence elsewhere, they can continue the specified vaccinations at the new residence or temporary residence's planned immunization outpatient clinic with the vaccination certificate. After the child's basic immunizations and booster shots are completed, parents should keep the vaccination certificate for future verification when the child enters daycare, school, military service, or travels abroad.
(3) Health Check-up Card for Children Aged 0-3
In the first month after the baby is born, the responsible hospital from prenatal check-ups will send a doctor for two home visits. They will weigh the baby, assess growth, teach new mothers how to massage and exercise the baby's muscles, etc. The doctor will remind the mother to take the baby for a physical examination at 30 days old.
At the full-month physical examination, the hospital will issue a "Health Check-up Card for Children Aged 0-3," specifying which stages require medical check-ups. Mothers should bring their babies for appointments according to the scheduled dates. Doctors will create a health check-up file for the baby. Each check-up will involve examining the baby's feeding habits, tooth growth, height, weight, head circumference, chest circumference, etc., and regular blood hemoglobin and trace element tests. Doctors in Tianjin will provide feeding guidance based on the baby's individual needs.
