Provide a set of materials for the insured including solutions, steps, and preventive guidance. Notify the three major credit bureaus and relevant financial institutions about the insured's identity or account theft information. Assist in providing a report to the police department. Timely assist in placing the stolen insured's credit data base and related accounts on alert signals as appropriate. Insurance benefits will be paid according to the standard for insured persons receiving treatment at contracted medical network hospitals and clinics: (1) Insured person receives treatment at contracted medical network hospitals and clinics; (2) Emergency treatment; (3) No contracted medical network hospital or clinic within fifty miles of residence, insured person receives treatment at non-contracted medical network hospitals and clinics.

Identity Theft:

During the insured period of this service, if the insured encounters identity theft (such as driver's license, etc.) for the first time, and the thief uses illegally obtained identity documents to apply for accounts, establish fake identities, and engage in other criminal activities, the insured will receive service assistance.

Twenty-four-hour doctor, clinic, and hospital medical payment review. Twenty-four-hour service for locating medical facilities. Monitoring patient condition, arranging communications between patients, family members, doctors, employers, consulates, or embassies. Arranging emergency medical evacuation. If the insured person's condition or injury is severe and life-threatening, upon recommendation by our approved doctor and prior approval by American International Assistance Service Company (Travel Assist), our company may pay for the transportation costs of a relative requested by the insured person to visit bedside. Visitation Transportation Costs

Pre-existing conditions. Pre-existing conditions refer to: According to this plan, any injury, illness, or condition diagnosed, treated, or prescribed medication by a practicing doctor within six months before the policy takes effect. State or federal government regulations take precedence. Pre-existing conditions will be excluded from coverage for six months after this plan takes effect. Continuous insurance is required for the insured person to qualify for pre-existing condition benefits. Pregnancy means conception must occur after the insured person's insurance takes effect. Costs exceeding normal, reasonable, and customary expenses. Free services usually provided by the school's medical center, infirmary, hospital, or an institution employed by the school; or services that could have been obtained at the student medical center, infirmary, or hospital but were sought elsewhere, except for emergencies. Costs for wearing glasses, contact lenses, hearing aids, or related prescriptions and examinations aimed at correcting vision, radio frequency keratotomy or laser eye surgery; injuries resulting from participating in riots; accidental injuries caused by flying airplanes or aircrafts, except passengers who purchased tickets and are riding on regularly scheduled flights operated by airlines. Persistent injuries or infections contracted during service in any country's military; treatment for mental or neurological disorders, unless otherwise specified; unnecessary treatments or surgeries, unless otherwise specified; treatments provided by government hospitals, except where laws stipulate that medical costs must still be paid without insurance; costs incurred after the insured person's insurance terminates; congenital conditions, except where there are special provisions for newborn infants; costs resulting from experimental or investigative services or equipment: including treatments, procedures, facilities, devices, drug usage, apparatuses, or equipment; professional services provided by relatives of the insured person or anyone residing with the insured person; costs for services and equipment: a) not necessary for treating injuries and illnesses; b) not recommended by the attending physician; routine health check-up costs outside of newborn infant care; dental care. Except for treatment due to accidental damage to natural teeth and temporomandibular joint dysfunction (TMJ); costs related to foot diseases, foot injuries, flat feet, corns, calluses, hallux valgus, or toenails; cosmetic or plastic surgery costs, except those necessitating medical care within 24 hours due to accidental injury; diagnostic or surgical costs related to infertility, except those caused by injuries or illnesses; costs for contraception, sterilization, or restoration of fertility, including process and equipment costs; costs covered by: occupational benefit plans, workers' compensation acts or similar laws, automobile medical payments or no-fault plans, public assistance programs, government programs, or any other valid and recoverable insurance; declared or undeclared war or military actions; implementation or intent to commit attacks or crimes, except self-defense behavior; suicide or self-mutilation or any such attempt while conscious or unconscious; claims submitted due to participation in: inter-school or professional sports competitions, contests, speed races, free diving, skydiving, mountain climbing (typically using ropes and guides), parachute water skiing, glider flying, bungee jumping, piloted sleds, snowmobile or off-road vehicle travel, riding other two-wheeled or three-wheeled motorcycles, private aviation travel, including balloon rides and ultralight aircraft; costs incurred due to intoxication or drug use by the insured person, except for medications taken following a doctor's prescription; costs incurred due to traffic accidents caused by driving vehicles not within the scope of the insured person's driver's license (does not apply if the insured person is a passenger); medical costs: circumcision, tubal ligation, vasectomy, mastectomy, breast augmentation; gender reassignment surgery, orthognathic surgery (including mandibular setback surgery); learning disabilities, smoking cessation, hair removal, hair transplantation or regeneration, organ transplantation; pregnancy or childbirth of dependents supported by the insured person; medical costs occurring in the insured person's nationality location exceeding $1,000 or occurring more than 30 days after entering the nationality location.

Hospital fees, nursing fees, and other related service fees, including professional service fees, but excluding personal nature service fees unrelated to medical care; additionally, hospital fees cannot exceed the average semi-private room rate of the hospital. Doctor diagnosis fees, treatment fees, and surgeon operation fees. Anesthesia drug usage and management fees. Medicines, X-rays, laboratory tests, radium therapy, radioactive isotope treatment fees, oxygen, blood transfusions, iron lungs, and medical treatment fees. Physical therapy fees. Must be prescribed by a doctor for treating a specific disability and implemented by a licensed physical therapist. Prescribed medicines by internal medicine or surgeons, topical or bandaging drugs. Dental treatment fees: This plan pays for treatment costs due to accidental damage to originally healthy natural teeth according to the accidental injury payment standard. Maximum payment per tooth is $250 USD, and maximum payment per accidental injury is $1,000 USD. Abortion: According to other disease payment standards, this plan pays up to $500 USD for insured persons undergoing abortion. Chiropractic fees: If chiropractic care is indeed needed, this plan pays no more than $35 USD per session, up to three times per week; the annual maximum payment is $1,000 USD.

Inpatient or outpatient treatment, this plan pays the lesser of the following two amounts: a) Normal, reasonable, and customary expenses incurred within the first thirty days after hospitalization each insurance year; b) Ninety percent of normal, reasonable, and customary expenses, up to a maximum of $5,000 USD. ($10,000 limit for international students studying in the U.S.)

Account Takeover:

During the insured period of this service, if the insured encounters credit card theft, bank account cards, ATM, telecommunications accounts, and loan limits theft for the first time, the insured will receive service assistance.

Eligibility Requirements

All students holding valid visas to enter the U.S., under the age of sixty-six, enrolled in primary, secondary, or higher education institutions in the U.S. can participate in the International Student Health Insurance program. Students registered in U.S. schools, under the age of sixty-six, studying courses in countries outside the U.S. (excluding their home country). Dependants (spouse and/or children under eighteen years old) need to enroll within thirty-one days of the insured person's enrollment, or on the day of birth, legal adoption, marriage, or arrival in the study country. Dependants can only enroll after the insured person has enrolled.

Policy Duration

The effective date of the insurance, if the application form has been submitted and accepted along with the required premium payment, the policy will take effect at 12:01 A.M. on the latest of the following dates: the effective date of the main policy; the effective date designated by the insured person on the application form; or the date the completed application form and correct premium amount are received by the plan administrator. This plan can be continuously renewed as long as the student meets the eligibility criteria. Termination date of the insurance, the earliest of the following dates at 11:59 P.M.: the expiration date specified by the paid premium; the termination date of eligibility; the termination date of the main contract. Termination date of the dependant's insurance, the earliest of the following dates: the expiration date specified by the paid premium for the dependant; the termination date of the dependant's eligibility; the termination date of the insured person's insurance.

If an accident occurs during the policy period requiring emergency transfer to the nearest medical facility with comparable equipment or return to the country of origin, this policy will cover all insured costs, up to a maximum of $500,000 USD. This emergency transfer must be determined by a legally qualified and licensed doctor as necessary due to the severity of the injury or illness. This emergency transfer also requires your or your representative's consent. All evacuation procedures must be arranged by American International Assistance Service Company (Travel Assist).

Within the policy period, if the insured person dies due to an accident or illness, the insurance payment includes all reasonable costs for embalming and repatriation of remains back to the country of origin, up to a maximum of $100,000 USD. All procedures must be arranged by American International Assistance Service Company (Travel Assist).

To avoid interruption of insurance, full premium payments must be made and a complete form submitted within thirty days of the end of this insurance in the new insurance year.

According to this plan, the insured person must verify compliance with the original eligibility requirements when renewing insurance for the next year. Additionally, the total payment for each accident or illness must not exceed the maximum payment amount specified by this plan for each accident or illness.

Under the following circumstances, the insured person may be compensated for losses listed in the table directly caused by an accident: 1) The bodily harm or permanent harm suffered by the insured person is directly caused by an accident, unrelated to disease, physical ailment, or other causes, and the accident occurred within the policy period outside the country of origin; 2) The loss appears within 365 days after the accident.

Due to any fatal or non-fatal injury losses caused by the following reasons: a) Disease, pregnancy, childbirth, miscarriage; bacterial infection, except bacterial infection caused by inflammation of accidental wounds; b) Hernia.

Any fatal or non-fatal injury losses caused by the following reasons: a) Self-inflicted or attempted self-inflicted injuries while conscious or unconscious; b) Declared or undeclared war or military actions, or participation in any country's military, naval, or air force organization; c) Any aircraft pilot, crew member, or passenger, except ticketed passengers on regular flights; d) Injuries caused by being under the influence of drugs, including alcohol-induced unconsciousness or anesthesia; but taking these drugs as prescribed by a doctor and not exceeding the prescribed dosage is exempted.

For medical expenses incurred by the insured person due to accidental injury or illness, we will provide payments to the insured person in proportion (see the table below), but not exceeding the maximum limit. Within fifty-two weeks from the date of the accidental injury or illness or the first occurrence of expenses, the cumulative maximum payment per accidental injury or illness is A: $250,000 USD B: $300,000 USD. The insured person should receive treatment within thirty days after the accidental injury occurs.

Covered Payment Scope

* Deductible portion fees can be waived if the insured person receives initial treatment at the school's healthcare center. Otherwise, the insured person must pay the deductible portion fee. The insured person's deductible will not exceed $250 USD per insured year. If the school does not have a healthcare center, the insured person must seek treatment at a contracted medical network hospital or clinic to waive the deductible portion fee.
